Account Set-Up

PLEASE NOTE:  Presently the Intrinsic Hepcidin IDx^TM Test does not have a CPT code and is not covered by insurance.  Thus the cost of the Intrinsic Hepcidin IDx^TM Test will be billed directly to the patient or institution.  We are working to have the cost of the Intrinsic Hepcidin IDx^TM Test covered by all major insurance providers. All other tests offered by IntrinsicDx are reimbursed by all major insurance providers.

Prior to sending samples:

1. Electronically or manually complete the Client Registration Form.
   1. All fields on the form are required.
   2. Form must be signed by an individual with signature authority for the institution.
2. Fax or email completed form to 858-459-7777 or clientservices@intrinsicdx.com.

To complete forms electronically, Adobe Reader is required.  A free copy of Adobe Reader is available here >>.

After receipt of the first sample, IntrinsicDx will provide an account number for future submissions and payments.
